Omitted Assessments Exemptions Applications

Omitted Assessment Notice(s) has been issued to the property listed on the enclosed notice. Applications for the Homeowner and Senior Citizen Exemptions are included in this notice.

Eligible properties are single family homes, condominiums, cooperatives, or apartment buildings of six units or less which were occupied by the current or previous owner(s) as their principal residence as of January 1 of the tax year in question. If the property meets these qualifications, please complete the questionnaire, sign the form, and return it to the Cook County Assessor’s Office as soon as possible.
